It will also help them to understand how to reduce their chance of having a heart attack or a stroke.

This toolkit will highlight your patient's options and choices. It will show you both how much benefit you are likely to get with each option. The toolkit will also share the possible downsides of each option with you both.

After using this toolkit, your patient will be better prepared for a shared decision making conversation with their doctor, or nurse. Your patient will be able to decide whether altering their lifestyle, with or without taking a regular medication, might be right for them. This toolkit will not make a medical diagnosis. The toolkit will guide your patient, and you, to choose options that are right for your patient.
